The authors present a comprehensive system of safety strategies and interventions aimed at managing patient safety daily and enhancing it over the long term. These strategies are relevant across all levels of the healthcare system, from frontline care to regulatory governance. Despite advancements in patient safety, a broader vision is necessary, one that encompasses the entirety of the patient’s journey. The authors emphasize the importance of viewing safety from the patient’s perspective, considering its management in various contexts, and developing a strategic vision that frames patient safety as risk management over time. While many safety improvement strategies focus on enhancing reliability and optimal care, healthcare will always face pressures, necessitating methods to manage safety under challenging conditions. There is a need for strategies that emphasize detecting, controlling, managing, and responding to risk. Approaches for managing safety in standardized environments differ significantly from those required in dynamic clinical settings. Supported by the Health Foundation, an independent charity dedicated to improving health and healthcare in the UK, this work aims to foster a healthier population through high-quality, accessible healthcare. The Foundation conducts policy analysis, funds innovative practices, and emphasizes the connection between frontline knowledge and research to inform effective policymaking.
