The clinical management of functional disorders of the upper and lower digestive tract has become a major challenge both at the level of primary care medicine and in specialist settings. Not only can patients often not be given a sufficient explanation of the cause of their symptoms, but complete symptom relief is very difficult to achieve with the currently available therapies. The aim of this issue is to provide an update on current facts and trends including clinical assessment and novel diagnostic technologies. Moreover, the currently available therapy principles as well as prospective ones are presented critically, including the management of dyspepsia and treatment options for functional dyspepsia. Apart from presenting current aspects of functional abnormalities associated with abdominal symptoms, this publication will also stimulate the awareness of the complexity of the visceral sensory network involved in this heterogeneous disease complex. This publication provides a wealth of information for both the practicing gastroenterologist as well as for the scientist concerned with basic mechanisms.

    • The overall prognosis for patients with gastric cancer remains poor as tumors are often diagnosed at an advanced stage, despite new developments and technical advances in endoscopic diagnosis and therapy. This publication brings together leading experts in the fields of basic research and clinical management of gastric cancer, to share their expertise with the clinical community. State-of-the-art reviews discuss the pathogenesis of and the most important risk factors for the development of gastric cancer, potential benefits of chemoprevention and Helicobacter pylori eradication. Moreover, new diagnostic modalities and surgical therapy, including the potential benefits of neoadjuvant and adjuvant therapies, are presented. As most patients require palliative therapy because of locally advanced or metastasized cancers, several reviews are dedicated to advances in the chemotherapy of advanced gastric cancers and peritoneal metastasis. Finally, future approaches towards a molecular diagnosis and therapy are described. Those involved in the management of gastric cancer patients will find the information presented in this publication of utmost help and will benefit greatly for their daily routine

This issue of Digestive Diseases provides an update and progress report on the management of patients presenting with complications of portal hypertension. If interventional radiological and endoscopic as well as pharmacological treatment options fail, liver transplantation is the ultimate rescue which relieves portal hypertension, prevents variceal re-bleeding, and prevents ascites and encephalopathy by restoring liver function. The complex issues involved in the management of portal hypertension are presented in this publication in a complete and clear way, providing a wealth of information for the clinician.

    • A critical appraisal of recent achievements What at the beginning of the last century started as a struggle to develop new devices for the endoluminal exploration of the digestive system has resulted in unexpected achievements in the field of advanced high-tech endoscopy such as interventional endoscopy and refinements in diagnostic resolution. This publication reviews some of the established major progress which has recently been made with regard to diagnostic and interventional endoscopy, including methods that are currently becoming part of today's clinical management. A critical evaluation of these methods is, however, mandatory as the enthusiasm for new technologies may result in their being introduced precipitately, without thorough prior evaluation in the form of adequate clinical trials: only with the help of such studies can individual applications be optimized. Last but not least, the importance of parallel instruments such as a self-administered symptom questionnaire to assess subjective manifestations and needs is stressed. This critical update on some of the latest issues in diagnostic and interventional endoscopy presents a valuable source of information for those working in clinical practice.
