Set against the backdrop of the Philippine-American War, the story follows José R. Lugay, a young medic whose aspirations are shattered by conflict. After marrying his childhood sweetheart, he faces the harsh realities of war and struggles to find work as a doctor in Manila. Relocating to the impoverished town of Guiuan, he eventually gains recognition as a Congressman. However, his political success prompts a crisis of identity, as he grapples with the choice between a life in politics and his original dream of being a distinguished doctor.
