The introduction of lasers and advances in fiberoptic technology have greatly enhanced minimally invasive surgery. This work aims to highlight the capabilities and benefits of lasers in urological surgery, making this technology more accessible. It encapsulates 25 years of laser research and clinical application, beginning in 1972 with studies on laser tissue interactions and fiberoptic endoscopic techniques, leading to the first clinical applications in 1975 and 1976. Early developments included laser lithotripsy in the 1980s and a research program in Lubeck that produced a "smart" laser lithotriptor (Lithognost) by the late 1980s. Recent innovations in photodynamic diagnosis using locally administered photosensitizers are set to redefine the role of the Nd:YAG laser in treating bladder cancer. Notably, interstitial laser therapy for prostatic hyperplasia and potentially certain types of prostatic cancer is an exciting development. Additionally, a technique for laser treatment of schistosomal bladder lesions, first described in 1986, has been established. This work is dedicated to the pioneers of laser medicine and the many colleagues and research assistants who have contributed to this journey, whose names are documented in over 300 publications.
